UIL Participation Forms

The updated Physical/Medical History form for the 23-24 school year is attached to this page.

 

The completion of this physical form is required for incoming 7th/9th/and 11th grade students by the UIL prior to participation in any athletic activity. The Medical History Form is required for all students every year. A hard copy of these forms must be turned in or emailed to Coach Woodard or Chase Remini.
