UIL Participation Forms

The updated Physical/Medical History form for the 24-25 school year is attached to this page.

 

The completion of this physical form is required for incoming 7th/9th/and 11th grade students by the UIL prior to participation in any athletic activity. The Medical History Form is required for all students every year. A hard copy of these forms must be turned in or emailed to Coach Woodard or Chase Remini.

 

All other UIL and West ISD forms are online and can be signed electronically on our RankOne page. The parent link is pasted here.....The 24-25 online paperwork will not be ready until at least May 1
